What You'll Bring to Our Team

Essential Qualifications:

  • Current Colorado PT license in good standing

  • 5+ years clinical experience (home health strongly preferred)

  • 2+ years in a mentorship or supervisory capacity

  • Expertise in geriatric rehab, post-surgical recovery, and complex mobility cases

  • Proven outcomes with Medicare quality measures

Your Advanced Skillset:
✔ Advanced manual therapy techniques
✔ Comprehensive fall risk assessment and prevention
✔ Wound care collaboration (with nursing team)
✔ Family/caregiver education strategies

Your Impact Will Include:

Clinical Excellence (70%):

  • Manage a specialized caseload of 18-22 patients weekly

  • Provide cutting-edge interventions for:

    • Post-hospitalization recovery

    • Neurological rehabilitation

    • Joint replacement protocols

    • Chronic condition management

Leadership (30%):

  • Mentor 3-5 staff PTs through:

    • Monthly case reviews

    • Joint visits (2-3x/month)

    • Best practice development

  • Participate in quarterly quality improvement initiatives

  • Serve as clinical liaison with physician partners
