Essential Qualifications:
Current Colorado PT license in good standing
5+ years clinical experience (home health strongly preferred)
2+ years in a mentorship or supervisory capacity
Expertise in geriatric rehab, post-surgical recovery, and complex mobility cases
Proven outcomes with Medicare quality measures
Your Advanced Skillset:
✔ Advanced manual therapy techniques
✔ Comprehensive fall risk assessment and prevention
✔ Wound care collaboration (with nursing team)
✔ Family/caregiver education strategies
✔ OASIS documentation expertise
Clinical Excellence (70%):
Manage a specialized caseload of 18-22 patients weekly
Provide cutting-edge interventions for:
Post-hospitalization recovery
Neurological rehabilitation
Joint replacement protocols
Chronic condition management
Leadership (30%):
Mentor 3-5 staff PTs through:
Monthly case reviews
Joint visits (2-3x/month)
Best practice development
Participate in quarterly quality improvement initiatives
Serve as clinical liaison with physician partners
Compensation & Benefits:
$105,000-$135,000/year (based on experience and caseload complexity)
$5,000 Sign-On Bonus (for full-time commitment)
$3,000 Annual CEU Budget + 3 conference days
